1. Elementary Graduate

2.Certificate of good moral character

3. Form 137 (permanent record)

4. Birth Certificate (NSO)

5. Marriage Certificate of Parents (original/photocopy)

4. Medical Certificate

7. Baptismal and Confirmation Certificates

8. 2x2 picture (3 copies)

9. 1x1 picture (3 copies)
